How to respond
The honest answer is: you mostly don't. The correct response to being left on read is to close the app and go do something else. Most guys can't do this because the read receipt feels like an open loop their brain refuses to close. That feeling is the thing to fix, not the text situation.
How to handle being left on read

The honest part
Read receipts aren't doing this to you. Neediness is. A guy with genuine options and a full life glances at a read receipt, thinks "she'll reply when she replies," and goes back to whatever he was doing. The timestamp only becomes a problem when one girl's response time has too much weight in your head. That's an abundance problem, not a texting problem. Fix the abundance problem, and the read receipt becomes exactly what it is: a delivery confirmation, not a verdict.
